Our grants are given on a monthly basis to fund a variety of things, from outings to basic living expenses to physical assets. Read below for more information on our guidelines. For any questions, please email info@momentsfoundation.org.
Grant Guidelines
What We Fund
Moments Hospice Foundation funds end-of-life wishes for patients in hospice care residing in greater Minnesota, Wisconsin and Iowa. Grant requests range between $100-$2,000 per patient and must fall in the following categories:
- Basic Needs such as clothing, food, gas or basic household needs
- Daily Living Expenses such as rent or utility bills
- Physical Assets such as books, furniture or technology (TV, DVD, iPad) for hospice home
- Social Outings such as a sporting event, restaurant or concert with loved ones (pending physician approval)
- Family Visits such as bringing family in from out of town or catering a family meal
Requests Outside Our Scope
- Items that are not at the sole request of the patient
- Improvements or construction to the family or hospice home
- Contracted programs, services or staffing
- Motorized vehicles
Grant Application Process
- Available below or upon request via email or US mail
- Accepted annually on a rolling basis
- Are accepted once per individual in their lifetime
- Award is based upon meeting eligibility requirements and available funds as determined by the Grant Committee
- The Grant Committee reserves the right to contact patient’s hospice care worker(s) to review application, request additional information or clarification prior to award
Grant Awards Timeline
- Applications are reviewed by the Grant Committee on the first Friday of each month
- Applicants will be notified of their status by the 15th of each month
- Upon award, Moments Hospice Foundation will purchase items/pay expenses on behalf of the grantee; no cash value is distributed to the grantee
- Grants are to be used within six months of award
- The Grant Committee reserves the right to meet more frequently, on a case-by-case basis, if needed based on applicant life expectancy
