As people spend their final time on this earth, Moments Hospice Foundation does its best to ensure they can pass with peace, comfort and happiness by funding end-of-life wishes. Patients in hospice care residing in greater Minnesota, Wisconsin and Iowa are eligible to apply. Grant requests range between $100-$2,000 per patient.
Hospice services are covered by Medicare, Medicaid and most commercial insurance plans. However, for those with no insurance or no ability to pay privately and that need the quality care, comfort and dignity deserved at end-of-life, we aim to provide scholarships for approximately 1,200 free hospice stays to patients per year.
We Honor Veterans is a national program to empower hospices and community organizations to meet the unique needs of America’s veterans and their families to be guided toward a more peaceful ending. We are proud to be a participating partner, and one of the few in Minnesota, where we can offer many benefits and programs to help our veterans enhance their end-of-life.
Through Virtual Reality Visits, we are able to bring the outside world in to someone going through the dying process. With the ability to “live again”, a Virtual Reality Visit can bring a patient an experience they had once in their life, or one they always wanted to have but never got around to doing. We make it a reality once more – a visit to the symphony or a trip to Washington D.C. to visit the historic sites. All of this from the comfort of their home.
There are a multitude of studies that show how virtual reality can help individuals suffering with dementia, depression and simulate an active lifestyle. With the proper equipment, staffing, training and volunteers, we can bring the benefits of virtual reality to hospice patients to create end-of-life comforts and enjoyment.
Moments Hospice Foundation enriches the experience for patients and veterans to create lasting end-of-life memories.
